New patient information & forms
We welcome new patients at Novant Health Today's Pediatrics. Whether you are seeking pediatric care for your new arrival or older child, our providers are eager to help you meet your goals for good health.
To plan for your first visit, please call our office during business hours, to allow us to collect information that is needed ahead of your appointment. Please arrive at least 15 minutes early on the day of your appointment and bring these items:
- A photo ID
- The insurance card covering your child
- Your method of copayment or payment
- Any prescription or over-the-counter medications you child is taking, including vitamins and supplements
