School, camp & sports physicals

Before your child attends school or camp, or participates in a sport or other activities, a physical exam might be required. This is recommended to make sure your child can safely participate in the activity without causing harm to himself or others.

Call our office to schedule a school, camp or sports physical as soon as you know one is required. Sometimes these exams can be completed at the same time as your child's annual physical exam. Bring with you any forms that need to be completed by our providers; we will not have access to these forms in our office. Verify coverage with your insurance company before your appointment, as these visits often will not be covered by a medical plan.
